Injection Sites

Abdomen

Subcutaneous fat of the abdomen, at least 2 inches from the navel. The most commonly preferred site.

  • Inject in the fatty area around (but not in) the belly button
  • Stay 2 inches away from the navel in all directions
  • Lift a skin fold with thumb and forefinger if needed
  • Rotate within a zone: e.g., upper-left, lower-left, upper-right, lower-right, cycling weekly
  • Avoid scar tissue, stretch marks, and bruised areas

Thigh

Front or outer thigh, mid-way between hip and knee.

  • Use the middle, outer front area of the thigh
  • Avoid the inner thigh (too close to vessels and nerves)
  • Pinch the skin before injecting if thigh fat is minimal
  • Alternate left and right thighs each week
  • Good for patients who find the abdomen uncomfortable

Upper arm

Outer, posterior aspect of the upper arm (triceps region).

  • Usually requires a partner or caregiver for injection
  • Use only the back of the arm — avoid the deltoid muscle
  • Difficult to self-inject correctly; use abdomen or thigh if injecting alone

Storage Instructions

  • Store pen in refrigerator until first use
  • After first use: up to 56 days at room temperature or refrigerator
  • Never expose to heat above 77°F or freeze
  • Keep the pen cap on when not in use

Where is the best place to inject semaglutide?

The abdomen is most commonly recommended because it's easy to self-inject and has good subcutaneous fat. All three approved sites (abdomen, thigh, upper arm) are equally effective — choose what's most comfortable for you and rotate consistently.

How far should I inject the needle?

Semaglutide is a subcutaneous (under the skin) injection, not intramuscular. Most auto-injection pens handle depth automatically. If using a syringe, inject at a 45° angle if skin folds are thin, or 90° if skin folds are adequate.

Can I inject semaglutide in my arm by myself?

It's possible but difficult. Most people find it easier to use the abdomen or thigh for self-injection. If the upper arm is your preferred site, consider having a partner or caregiver administer the injection.

What happens if I inject in the same spot every week?

Injecting repeatedly in the same spot can cause lipodystrophy — lumpy, hardened fatty tissue. This can impair drug absorption and cause aesthetic changes. Consistent site rotation (covering at least 3–4 different spots) prevents this.
